Mission Statement

The Student Health Office promotes optimal student education by providing: health screening, health education, health care and first aid for students at Koogler Middle School. We believe healthy students are better learners.

Mrs. Bernard

Mrs. Bernard is the health aide for Koogler's nurse’s office. She has been at Koogler for 14 years. In the health office she attends to students with health issues ranging from minor first aid to diabetes care. She is a Certified Medication  Aide and is continuing her education in the nursing field by attending San Juan College to receive her RN licensure. Mrs. Bernard enjoys photography, quilting and 4-wheeling in her spare time.


The State of New Mexico requires current immunizations for the following :

Tetanus/Diphtheria/Pertussis, Polio, Measles/Mumps/Rubella, Hepatitis B, Varicella. The newest  immunizations change is the Tetanus 5yr booster given in the form of Tdap. If a medication or treatment has been prescribed for your child to use during the school day, please meet with or call the school nurse.  School policy allows prescription medications to be administered at school when accompanied by the Aztec School District Medication Authorization Form which has been filled out and then signed by the physician or primary care provider.

Health Issues

Let the nurse know if you or your physician have any concerns with the health or well being of your child.  This information, if educationally relevant will be shared with the necessary school staff in a professional and confidential manner.  The information must be updated yearly on the Student Health History Form at the time of registration


Vision screening

Students new to the district, all 8th grades, all students referred for special educational testing have routine vision screening. Screening  is also conducted on students referred to the nurse by parent, teacher or student self referral. Individuals are screened twice (2nd time by school RN) before being referred for complete vision evaluation by the family eye care specialist.

Hearing screening

The school RN conducts hearing screening on all students who have been referred for special educational testing, students new to the district and students referred by parent, teacher or student self-referral. Second screening failures will be appropriately referred for medical/audio- logical follow-up.
